Scenario A nonprofit hospital located in a suburban region serves a population of approximately 150,000 people, including a high proportion of low-income families and older adult residents. The hospital must conduct community health and outreach programs to maintain its nonprofit status, as required by IRS regulations. The hospital recently conducted a community health needs assessment (CHNA) that identified two critical issues: (1) the rising prevalence of type 2 diabetes and cardiovascular disease among residents in lower-income neighborhoods and (2) barriers to accessing healthcare, such as transportation challenges, language barriers, and lack of trust in healthcare institutions among minority groups. In response, the hospital launched a community health outreach program (CHOP). One of the primary initiatives is a series of free diabetes prevention workshops offered in partnership with local community centers.
