Part III: Direct Community Care Activity (Unit 5)
In the final part of the Population Health Assessment and Prevention Project, a direct care component will be incorporated.
For part 3:
• Create a presentation that summarizes the community health data, key health issues, and connection to the national health initiative identified in Part II. In addition, propose local action steps or interventions based on the goal included in the Healthy People 2030 initiative.
The purpose of this presentation is to inform, raise awareness, and equip the group or stakeholders with the information needed to lead strategies, develop policies, or implement a community action plan to address the identified community health issue in alignment with the national campaign.
For example: 1) invest in community infrastructure that supports active living, such as safe walking/biking trails and public parks; 2) encourage local employers to adopt wellness initiatives, such as scheduled activity breaks; 3) collaborate with community fitness facilities to provide reduced-cost memberships or free trial programs for adults.
Step One: Create Presentation
Create a presentation using PowerPoint, Canva, Google Slides or similar software.
• Review Microsoft Office Tips for Creating an Effective Presentation.
• Review additional tips for Making an Effective PowerPoint Presentation.
Presentation slide requirements: Create a slide presentation including the following slides with bulleted content and graphics, as applicable:
• Title Slide: List an interesting title, your name, and the date.
• Community Health Issue: Identification of the key health issue in the community (include data to support the selection).
• Target Population: Describe the specific group of people within the community who are most affected by or at risk of the health problem being addressed.
• Rationale: A brief summary of the community health data related to the key health issue.
• National Health Initiative: Briefly describe the national health initiative.
• National Health Goal & Community Health Need: Describe the connection between the key health community issue and the Healthy People 2030 health initiative.
• Action Steps: Lists 3 evidence-based action steps or interventions that could be implemented in the community to address the key health issue. These steps should align with the goal from the Healthy People 2030 initiative. Use supporting in-text citations.
• Reference Slide: Include a minimum of 3 scholarly sources published within the last five (5) years. One of the sources must be from:
o Centers for Disease Control and Prevention (CDC);
o National Institutes of Health (NIH);
o Agency for Healthcare Research and Quality (AHRQ).
• Presentation Design and Formatting:
o The presentation should be organized, creative, and professional.
o Use contrasting uniform font and background colors with appropriate font sizes to enhance readability. (Review the Microsoft resource for Making PowerPoints Accessible.)
o Use short, bulleted points and limit the use of large number of texts.
o Use smart art, graphics, and quality images when appropriate to support or explain the content and reinforce the presentation.
Step Two: Deliver Presentation
Schedule a date to deliver your presentation to the chosen small group or key stakeholders prior to Week 5 of the term. During the presentation, be sure to:
• Dress professionally and present the content in a professional and concise manner.
• Be sure to wear your Galen College of Nursing student ID badge. Since this presentation is part of your role as a nursing student, not as a practicing RN, do not wear your work uniform or employee ID.
• Inform the small group or key stakeholder about the community health issue in a compelling way that highlights its significance and the need for immediate action.
• Clearly articulate the Healthy People 2030 initiative.
• Advocate for realistic, local action steps that align with the national health goal.
• Submit the presentation AND Part 2 of the Direct Care form. The direct care form requires that the primary stakeholder review and reflect on the presentation and provide contact information. This person MAY be contacted to validate the completion of the direct care component.
Step Three: Reflection
Write a 1 paragraph reflection of your presentation, and a 1 paragraph reflection on a sphere of care. Specifically:
• Use the template below to include a brief reflection (1 paragraph) by answering the following questions:
o After reflecting on the presentation, what would you do differently?
o What was the most important takeaway for the teaching provided?
o If you had unlimited time, what would you expand on and how?
o How would you incorporate feedback provided by the stakeholders/group to improve the overall presentation?
• Use the template below to include a brief reflection on one sphere of care. The Four Spheres of Care were developed by the American Association of Colleges of Nursing (AACN) as part of the Essentials, which guide how nurses are educated today. These spheres help organize the different types of care nurses provide across all settings. They were created to show how nurses support people at every stage of health. By understanding the four spheres, nursing students can better see the full range of how their care makes a difference in individual lives and in entire communities. The four spheres of care: 1) Wellness and Disease Prevention; 2) Chronic Disease Care; 3) Regenerative/Restorative Care; and 4) Hospice/Palliative Care.
o Write a 1 paragraph reflection by selecting one sphere that supports your proposed strategies. Consider how your presentation connected this sphere to the community’s needs and how it aligns with the Healthy People 2030 goal you selected. Reflect on how your knowledge of this sphere of care deepened your understanding of population-based care.
For example: My presentation focused on the Wellness and Disease Prevention sphere of care, addressing adult obesity using the Healthy People 2030 goal “Overweight and Obesity.” The objective to reduce adult obesity supports community action plans aimed at decreasing health disparities. By adapting strategies like walking/biking trails, scheduled activity breaks, and fitness incentives, I gained a deeper understanding of how prevention, education, and policy change can improve population health.
